Appointments

There are two parts to registering with us, you will need to:
Complete a medical form AND have your first registration dental examination.
Following this you will become a registered patient with us.
The Medical form can be found under the tab Get in Touch then bottom right Medical History. Alternatively you can come into our Dental Health Centre and collect a form.
The form must be completed in full and then returned to us in person so we can then make your first registration examination appointment, We can not accept forms completed and returned via email or post.

All calls to and from Diss Dental Health Centre may be monitored and recorded for the purpose of quality management, training and customer support. If you would like to make an appointment, please contact our practice in person or by phone.

Cancellation

At Diss Dental Health Centre, we strive to establish and maintain good relationships with our patients. We provide quality treatment and dedicate time specifically for your appointment. If you do not keep your appointment, that time is lost and adds significantly to our patient appointment waiting times, therefore we can reallocate the surgery time.

We make every effort to keep our fees reasonable, but missed appointments can make this difficult. We appreciate you as a patient and in return we ask for the same consideration. We require 48 hours’ notice (2 business days) to cancel an appointment. This will give us adequate time to offer your appointment time to another patient who may need it. For repeated late cancellations and missed appointments, we’re sure you’ll understand that an appropriate fee will be applied to your account.

In an Emergency

We daily offer a small number of appointments during practice open hours for people who need urgent treatment, but do not have an appointment. This service is only for people with acute dental pain and it is recommended that you call at 8.30am when the practice opens.
If you have never seen one of our dentists or if you have not attended our Practice for a long period of time we are able to see you as an emergency patient, however, this would be on an independent fee.
Should you have a dental emergency outside of the practice opening hours, please contact NHS Direct 111 they will be able to advise you on the best course of action. This service runs in the evenings, weekends and all bank holidays.
